How does one create at custom map .txt file ?

You know ... the file describing the map mission and credits in plain text.

Have tried to make a file using notepad and placing in the /maps folder, but i does not get used when creating a game.

It works for cs afaik. For cs, whatever is in <mapname>.txt will show up on the objectives screen.

You'll also want to make a .res file for other players who might download your map through a server so that they download the map description.
