Hm, I've always textured the basics null/bevel first then do the detailing with actual textures.

I've had more troubles with this in Hammer 4 than I ever did with Hammer 3. I only recall having the issue in 3 when I had copied a large amount of brushes at a time, and even then, it only really happened when copying more complex geometry.

300x400 isn't even a valid resolution in the new version of the engine. Use 640x480.
Since they share content with eachother, you'll most likely have to uninstall ALL HL1 based games to fix it.
