It really depends on how you want to add it.
To add the laser to the weapon itself, you would need to do some modelling.
To have a laser pointer in the game, like the RPG or Opposing Force's Desert Eagle, you would need to do some coding.

Milkshape3D is one option yes. You can also use blender but the process is a little more long winded.
