Photoshop's perspective tool uses the image as a plane, not a 3d model really. You can do some nice stuff with it, but it's really only a plane. (This is really only CS3, not sure about basic CS3 either, cuz i use Extended)
I don't believe so, as images are 2d pictures, and therefore the sides and back wouldn't have actually been created.
Although, it might be possible if such a program created what it THINKS it would look like. Other than that, your image looks like an animated sprite for a 2d game. =D

up the quality? clean it up? uhmmm eye? whats that about? id imagine chaos more like buildings collapsing. but uhmm... yeah, i dont like the way the eye looks, its too choppy
only thing i would need is the clock... sure the ram and cpu usage would be nice... but... bleh. other than that, i have a clock behind me... thats relatively hard to see without spinning my chair 180, in which case, if im wearing my headset, i end up goofing and getting tangled @.@
as i said though. sounds frequency has always played me a bit. especially in a lower end older game such as half-life, you're much more likely to use a lower frequency than in Source.
Try messing with the sound file. Frequency, length, among others. Simply enough, do you have a file, or a bit of code misspelled? Even though you say the code is fine, it may not be. Like anything when looking for spelling, read it aloud to yourself until you notice an error. Long and unwanted, yes, but it's an extra review that MAY help.
