DVI vs. VGA thing: depends on the monitor. Doesn't make a noticable difference on mine, but it may on some.

You probably made some mistake when making your custom textures: invalid dimensions, wrong format, etc. Have a check.

Um, it's no different to having a static page valid. Just make sure that anything you push in with PHP is valid where you're putting it.
It hasn't, for most. They just have to run at whatever resolution their TV runs at. 576 x something for England. 1080 x something if you have an HDTV.
