ive wondered if this is done manually

when you put entities and you have to put them in the exact middle of something

an example would be trains, or elevators

You just have to figure it out manually. If it is a single brush the dead center is where the 'x' is on the 2d views.

If the entity is broken into several brushes then you should select the lot and check the dimensions at the bottom of the screen and half the height/length/width accordingly.

I assume your talking about an origin brush if concerning elevators and trains. In which case just put the origin block (i tend to make it 32x32x32 and place it in the center of the floor (the lower most brush)

If you need it in exactly the middle, try this:

Get the brush you need the entity to be in the middle of and select it. Hit Ctrl + E to snap all 2D views to the center of the brush. Then move the entity into the middle of the brush. Center the views while selecting the brush, and do it again while selecting the entity or origin. If your 2D views don't move at all while centering on either object, it is perfectly aligned in the center.
