A good deal of custom code I imagine. It's not standard functionality for Goldsource.

You'd have to speak to Magic Nipples or one of the SHAFT developers if you wanted to know exactly how they've done it. Assuming that is an actual background map and not just an image...

It can be made by using the Xash3D Engine. Vanilla Goldsource doesn't have that feature at all.

But as UrbaNebula said, ask Magic Nipples.

Xash3D is a custom build. It might be based on the WON version, but it's not the same thing.

No, that's Xash. That image was posted on both Moddb and their website with this description attached
If you had your eyes open for the first few seconds of the video, you can see what appears to be a hl2 style background map. Well if you guessed that then you are correct! Xash allows for map backgrounds for the menu. While it isn't chapter specific like hl2, the feature is very much appreciated, and we plan on taking full advantage of it in the mod.
Also, I don't think such a feature could be easily added to Goldsource (without access to engine source code).

I got one idea, but it'd be too much effort.

First of all, you'd basically launch a map as soon as you got into the game. Then a custom main menu would show up.
You'd likely have to use TriAPI there if you don't wanna mess with VGUI1 (and keep in mind, there is no access to VGUI2) to render main menu elements.

And then you'd have it. A frozen player with a main menu being rendered onto the view.
To change settings, you could easily modify the CVars via these custom menu elements, I believe.

The bad thing is, it'd require too much work, basically a complete replica of the main menu code.

So yeah, as others pointed out, you could use Xash3D.
